The Day of Remembrance is a day commemorating Antioch-Lithonia's deceased members.

Each name will be printed in the commemorative program.

Directions

1. Complete one sheet for each individual.
2. Please provide a brief statement about each person as a member of Antioch.
3. Additional forms are located at the Help Desk in the vestibule.
4. Place completed forms in the box label Day of Remembrance.
5. Each individual will be given only two lines.
6. Deadline for submission is January 30, 2019.

Example:
Reverend Matthew Mitchell, Antioch's first pastor. He pastored for one year, 1869 - 1870.
